Subscription plan
Ongoing medication management is provided under a $400-per-month subscription with a minimum three-month commitment. The first three months are prepaid at $1,200.
The subscription includes two appointments per month and direct access to Dr. Reyes via email, phone, and text. Please review the full subscription details for information about between-visit consultations, additional appointments, paperwork fees, cancellation, and discounts.
Individual appointments
Individual appointments that do not involve medication management are available outside the subscription plan at $500 per appointment. Examples include assessments of capacity to return to work, second opinions, and disability evaluations. Please contact the office to discuss your evaluation needs.
Insurance: Please be advised that our clinic does not accept insurance. We may collect insurance information for prior authorizations or lab order requests, but Dr. Reyes does not bill insurance for services provided. Superbills can be provided upon request for submission to your insurance company for possible out-of-network reimbursement.
